Work Permit

How do I complete a work permit?

All students 16 to 18 years old must get  a work permit from the school in order to work.

 

After you have been hired, print up a work permit form from here.

The student completes Section 1.  

The employer completes and signs Section 2.  The Employer also signs and dates Page 2.  

The parent/guardian fills out and sign Section 3  

Both pages must be taken to the Educational Services Center (Fremont Union High School District Main Office);

A district office employee will fill out Section 4 and issue the work permit. The District Office will process work permits on Tuesday and Thursdays, and they will be ready for Pick-Up on Wednesdays and Fridays.
